RESUMO

Proceeding from oneness to twoness is a psychological process of relating inter- and intrapersonally. This article links the perspectives of French psychoanalyst André Green's concepts of the dead mother and narcissism with Hester Solomon, British Jungian analyst writing on the 'as-if' personality. These concepts are elucidated with the composite example of a self-described sexually addicted man. His behaviours attempted to mask the shadows of melancholy, a fragile self, and the absence of self-animation from early emotional wounds. He did not know love or the other. André Green, French psychoanalyst, described feelings of misery, lack, and emptiness. The defence against relatedness arises from fears of replicating the original object losses. He delineated death narcissism and life narcissism as limiting relationships and creating the illusionary. Narcissus could not live if he knew himself. Immersion within singularity occludes relationship to the unconscious and the other, like Echo. Jung's concept of the transcendent function evolves from inclusion of the symbolic through listening to the language of the unconscious. Through the transference and countertransference, the former disowned and split-off others, secreted in the shadows of addictions, open relatedness to self, soul, and world.

Cet article met en rapport le concept de la mère morte du psychanalyste français André Green et celui de narcissisme dans les écrits de l'analyste Jungienne Britannique Hester Solomon, concernant la personnalité « as-if ¼. Ces concepts sont élucidés avec l'exemple d'un homme qui s'est décrit lui-même comme « addict sexuel ¼ (il s'agit d'un cas composé). Ses comportements visaient à masquer les ombres de la mélancolie, d'un soi fragile et d'une absence d'animation venant de blessures émotionnelles précoces. Il ne connaissait pas l'amour, ni l'autre. André Green, psychanalyste français, a décrit des sentiments de désolation, de manque et de vide. La défense contre le lien avec l'autre provient de peurs de retrouver les pertes de l'objet originel. Esses conceitos são elucidados com o exemplo composto de um homem sexualmente viciado autodescrito. Seus comportamentos tentaram mascarar as sombras da melancolia, do eu frágil e da ausência de autoanimação das primeiras feridas emocionais. Ele não conhecia o amor ou o outro. André Green, psicanalista francês, descreveu sentimentos de miséria, falta e vazio. A defesa contra a relação surge do medo de replicar as perdas originais do objeto. In this paper I discuss Jungian psychological work of the trauma and loss experienced in reaction to COVID-19 with a man who represents a clinical composite. The issues of precarity, a concept used by the philosopher Judith Butler, are combined with the notions of lack and absence of French psychoanalyst André Green. The psychological and societal situation of precarity aroused the man's childhood issues that were long repressed. The loneliness, isolation and death from COVID-19 mirrored his personal and the collective responses to the disaster from this global pandemic. He felt on the edge of collapse as what he knew of his world crashed and he found himself unable to cope. The subsequent Jungian work taking place through the virtual computer screen was taxing and restorative simultaneously for both analyst and analysand.


Dans cet article je discute le travail psychologique Jungien sur le traumatisme et la perte dont fait l'expérience un patient en réaction au COVID-19, patient qui représente un composite clinique. Les questions en lien avec la précarité, un concept utilisé par la philosophe Judith Butler, sont combinées avec les notions de manque et d'absence chez le psychanalyste français André Green. La situation psychologique et sociale de précarité a activé les problèmes de l'enfance de cet homme, réprimés depuis longtemps. La solitude, l'isolement et la mort liés au COVID-19 ont fait miroir avec ses réponses personnelles et les réponses collectives au désastre de cette pandémie globale. Il se sentit à la limite de l'effondrement alors que ce qu'il connaissait de son monde s'écroulait et il se trouva dans l'incapacité de faire face. Le travail Jungien qui s'ensuivit, au moyen de l'écran virtuel de l'ordinateur fut éprouvant et réparateur pour l'analysant et l'analyste simultanément.


En el presente trabajo, describo el trabajo psicológico Junguiano, con un hombre que representa un compuesto clínico, acerca del trauma y la pérdida experimentada en reacción al COVID-19. Cuestiones de precariedad, un concepto utilizado por la filósofa Judith Butler, son combinadas con las nociones de falta y ausencia del psicoanalista francés André Green. La situación psicológica y social de precariedad activa temas infantiles del individuo, hasta entonces reprimidos. La soledad, aislamiento y muerte del COVID-19 espejó la suya personal y las respuestas colectivas al desastre de esta pandemia global. Él se encontró en el borde del colapso debido a que lo que él conocía del mundo se quebró, y se encontró imposibilitado de hacer algo con esto. OBJECTIVE: Examine inhibition of dietary fat absorption with orlistat tablets (24, 36, 48, 72, and 144 mg) vs. 60-mg orlistat capsule. MATERIALS AND METHODS: 83 overweight/obese subjects randomized to 1 of 6 open-label treatments. Pre- vs. post-treatment fecal fat analysis was conducted. RESULTS: Mean percent fecal fat (60-mg capsule, 16.8%; 48-mg tablet, 16.5%) was similar (ratio of geometric mean and 90% CI: 60-mg capsule/48-mg tablet, 1.05 (0.79, 1.39)). Fecal fat excretion was ~2.5 times greater with 144-mg vs. 24-mg tablets. No new safety concerns emerged. CONCLUSION: Dietary fat excretion increases with increasing orlistat tablet dose.

We sought to establish the bioequivalence of 2 weight-loss aids: orlistat 27-mg chewable tablet and orlistat 60-mg capsule, measured pharmacodynamically as percentage fecal fat excretion. Two open-label, single-center, randomized, 3-period, 3-treatment crossover studies were conducted in adults with body mass index 25-33 kg/m2 . For each 9-day treatment period, subjects received orlistat 27-mg chewable tablet, 60-mg capsule, or 120-mg capsules (2 60-mg capsules) 3 times daily; a 2-day washout separated treatments. Primary bioequivalence analyses were based on 2 1-sided tests of the 90% CI of the ratio of geometric means using log-transformed data (study 1) and by the dose-scale method to calculate bias-corrected and accelerated 90% CI of relative bioavailability (f) using nontransformed data (study 2). Bioequivalence was established if 90% CIs fell within 0.80-1.25. In total, 48 and 144 subjects were randomized in study 1 and study 2, respectively. Bioequivalence between the formulations was established in both studies: study 1 ratio of geometric means of percentage fecal fat excretion was 0.96 (2 1-sided tests, 90% CI 0.87-1.06); study 2-point estimate of f was 1.09 (bias-corrected and accelerated 90% CI 0.98-1.22). Tolerability of the 27-mg tablet was consistent with the 60-mg capsule; mild gastrointestinal effects were most common.

The range of fault slip behaviors near the trench at subduction plate boundaries is critical to know, as this is where the world's largest, most damaging tsunamis are generated. Our knowledge of these behaviors has remained largely incomplete, partially due to the challenging nature of crustal deformation measurements at offshore plate boundaries. Here we present detailed seafloor deformation observations made during an offshore slow-slip event (SSE) in September and October 2014, using a network of absolute pressure gauges deployed at the Hikurangi subduction margin offshore New Zealand. These data show the distribution of vertical seafloor deformation during the SSE and reveal direct evidence for SSEs occurring close to the trench (within 2 kilometers of the seafloor), where very low temperatures and pressures exist.

In subduction zones, elevated pore fluid pressure, generally linked to metamorphic dehydration reactions, has a profound influence on the mechanical behavior of the plate interface and forearc crust through its control on effective stress. We use seismic noise-based monitoring to characterize seismic velocity variations following the 2012 Nicoya Peninsula, Costa Rica earthquake [M w (moment magnitude) 7.6] that we attribute to the presence of pressurized pore fluids. Our study reveals a strong velocity reduction (~0.6%) in a region where previous work identified high forearc pore fluid pressure. The depth of this velocity reduction is constrained to be below 5 km and therefore not the result of near-surface damage due to strong ground motions; rather, we posit that it is caused by fracturing of the fluid-pressurized weakened crust due to dynamic stresses. Although pressurized fluids have been implicated in causing coseismic velocity reductions beneath the Japanese volcanic arc, this is the first report of a similar phenomenon in a subduction zone setting. It demonstrates the potential to identify pressurized fluids in subduction zones using temporal variations of seismic velocity inferred from ambient seismic noise correlations.

This observational, cross-sectional study investigates cortical signatures of developmental dyslexia, particularly from the perspective of behavioral remediation. We employed resting-state fMRI, and compared intrinsic functional connectivity (iFC) patterns of known reading regions (seeds) among three dyslexia groups characterized by (a) no remediation (current reading and spelling deficits), (b) partial remediation (only reading deficit remediated), and (c) full remediation (both reading and spelling deficits remediated), and a group of age- and IQ-matched typically developing children (TDC) (total N = 44, age range = 7-15 years). We observed significant group differences in iFC of two seeds located in the left posterior reading network - left intraparietal sulcus (L.IPS) and left fusiform gyrus (L.FFG). Specifically, iFC between L.IPS and left middle frontal gyrus was significantly weaker in all dyslexia groups, irrespective of remediation status/literacy competence, suggesting that persistent dysfunction in the fronto-parietal attention network characterizes dyslexia. Additionally, relative to both TDC and the no remediation group, the remediation groups exhibited stronger iFC between L.FFG and right middle occipital gyrus (R.MOG). The full remediation group also exhibited stronger negative iFC between the same L.FFG seed and right medial prefrontal cortex (R.MPFC), a core region of the default network These results suggest that behavioral remediation may be associated with compensatory changes anchored in L.FFG, which reflect atypically stronger coupling between posterior visual regions (L.FFG-R.MOG) and greater functional segregation between task-positive and task-negative regions (L.FFG-R.MPFC). These findings were bolstered by significant relationships between the strength of the identified functional connections and literacy scores. We conclude that examining iFC can reveal cortical signatures of dyslexia with particular promise for monitoring neural changes associated with behavioral remediation.

It is well established that abdominal obesity or upper body fat distribution is associated with increased risk of metabolic and cardiovascular disease. The purpose of the present study was to determine if a 24 week weight loss program with orlistat 60 mg in overweight subjects would produce a greater change in visceral adipose tissue (VAT) as measured by computed tomography (CT) scan, compared to placebo. The effects of orlistat 60 mg on changes in total fat mass (EchoMRI-AH and BIA), ectopic fat (CT) and glycemic variables were assessed. One-hundred thirty-one subjects were randomized into a multicenter, double-blind placebo controlled study in which 123 subjects received at least one post baseline efficacy measurement (intent-to-treat population). Both orlistat-and placebo-treated subjects significantly decreased their VAT at 24 weeks with a significantly greater loss of VAT by orlistat treated subjects (-15.7% vs. -9.4%, P < 0.05). In addition, orlistat-treated subjects had significantly greater weight loss (-5.93 kg vs. -3.94 kg, P < 0.05), total fat mass loss (-4.65 kg vs. -3.01 kg, P < 0.05) and trended to a greater loss of intermuscular adipose tissue and content of liver fat compared with placebo-treated subjects. This is the first study to demonstrate that orlistat 60 mg significantly reduces VAT in addition to total body fat compared to placebo treated subjects after a 24 week weight loss program. These results suggest that orlistat 60 mg may be an effective weight loss tool to reduce metabolic risk factors associated with abdominal obesity.

This study compared the efficacy of 4 different hamstring-stretching techniques. Flexibility can be achieved by a variety of stretching techniques, yet little research has been performed on the most effective method. The 2 basic types include active stretching, in which range of motion is increased through voluntary contraction, and passive stretching, in which range of motion is increased through external assistance. The 2 types of active stretching include neuromobilization and proprioceptive neuromuscular facilitation (PNF). Our study aims to determine which type of stretching technique is most effective in improving hamstring length. One hundred subjects between the ages of 21 and 57 were enrolled in the study. Intrarater reliability of hamstring length measurement was performed using 10 subjects. All 100 subjects were included in a randomized controlled trial of 5 different groups comparing different hamstring-stretching techniques. Outcome measures, including hamstring length and perceived level of hamstring tightness, were recorded on all subjects initially, at 4 weeks, and at 8 weeks. After 4 weeks of stretching, there was a statistically significant improvement in hamstring length (p < 0.05) using active stretches as compared with passive stretches. From weeks 4 through 8, hamstring length for the active stretching groups decreased. After 8 weeks of stretching, the straight leg raise (SLR) passive stretch group had the greatest improvement in hamstring length. There was no correlation between hamstring flexibility and age, initial tightness, or frequency of exercise per week. Improvement in hamstring flexibility was greatest for the SLR passive stretch. Also, using PNF in the 90/90 active stretch provided better knee range-of-motion improvements than the 90/90 passive methods did.

OBJECTIVE: The study was conducted to provide information on how consumers would use orlistat 60 mg, especially in terms of product dosing, in a setting without physician supervision. METHODS AND PROCEDURES: A 3-month, open-label, naturalistic study was conducted in an over-the-counter (OTC) setting in 18 pharmacies. Consumers >/=18 years were allowed to purchase orlistat packages containing a bottle of orlistat 60 mg plus educational materials, which provided lifestyle information and tools to encourage successful weight loss. Data were collected at pharmacy visits and during telephone interviews at 14, 30, 60, and 90 days after enrollment. RESULTS: A total of 237 subjects purchased and used the product, and completed at least one interview. Most subjects followed the dosing directions and took two to three capsules per day with meals throughout the study. The majority of subjects took a daily multivitamin, as directed. Approximately, 80% of subjects used the educational materials and found them useful or very useful. Over the study duration, most subjects reported following a diet and 51% of subjects reported more frequent or longer exercise than at enrollment. Approximately, 80% of subjects indicated they were satisfied or very satisfied with the weight loss achieved; measured and self-reported relative median weight loss was approximately 5% after > or =60 days of using orlistat. Most common adverse events were gastrointestinal (GI), and majority of subjects did not interrupt or discontinue orlistat due to these GI events. DISCUSSION: These results demonstrate that orlistat 60 mg can be used appropriately and safely and with high consumer satisfaction without physician supervision or dietary counseling. Collectively, results indicate that orlistat 60 mg is an appropriate weight loss therapy in the OTC environment.

BACKGROUND: Lifestyle measures are considered the first line of therapy for treating overweight individuals, but many are unable to achieve a meaningful weight loss. OBJECTIVE: To determine the efficacy and safety of orlistat 60 mg, given 3 times daily, for weight loss in mildly to moderately overweight individuals. METHODS: A multicenter, 16 week, randomized, double-blind, placebo-controlled study was conducted in 391 overweight subjects at 20 US centers. The main outcome measure was change in weight from baseline to week 16; secondary measures included changes in body mass index, waist circumference, blood pressure, and fasting lipoprotein and glucose levels. RESULTS: Subjects in both groups lost weight over the treatment period; however, orlistat-treated subjects lost significantly more weight than placebo-treated subjects beyond 2 weeks of treatment. Weight loss from baseline to week 16 was significantly greater in participants receiving orlistat versus those receiving placebo (3.05 vs 1.90 kg; p < 0.001, intent-to-treat analysis). Orlistat-treated subjects who completed 16 weeks of treatment lost 4.8 +/- 0.35% (mean +/- SE) of baseline weight compared with 3.1 +/- 0.38% for the placebo group (p < 0.001). Orlistat-treated subjects, compared with those receiving placebo, also demonstrated a greater relative reduction in total (-4.4% vs 0.0%; p = 0.004) and low-density lipoprotein cholesterol (-7.2% vs -0.6%; p = 0.005) and both diastolic (-3.9% vs -0.5%; p = 0.001) and systolic blood pressure (-4.7% vs -1.8%; p = 0.004). Both groups showed a similar safety profile; gastrointestinal events were significantly more common in the orlistat-treated subjects. CONCLUSIONS: The use of orlistat 60 mg by mildly to moderately overweight individuals produced significant weight loss in conjunction with a reduced calorie diet and self-instructional materials. This amount of weight loss was associated with improvements in several weight-related risk factors. Orlistat 60 mg may be a useful adjunct to lifestyle measures and has the potential to contribute significantly to weight and risk factor improvement for overweight individuals.

The prognosis for renal allograft survival in a patient with BK virus-associated nephropathy is distressing because nearly 50% of affected grafts are lost. With these statistics in mind, attempting clinical intervention before BK virus disease develops seems appropriate. Noninvasive screening tools such as tests to detect decoy cells in urine can enable diagnosis of BK viral infection in its early stages. Although it is not entirely clear which patients with BK infection will have BK disease develop, it may be possible to intervene in this early warning period to prevent the further development of BK virus-associated nephropathy. We present a case of a 50-year-old man in whom asymptomatic BK viruria developed 6 weeks after he received a non-HLA matched kidney from a deceased donor. This BK viruria was accompanied, subsequently, by an unexplained increase in serum creatinine level 2 months after transplantation. Following a change in his immunosuppressive regimen as therapy for presumed renal allograft dysfunction associated with BK viral infection, the decoy cells in his urine disappeared and his creatinine levels stabilized.

INTRODUCTION: To conduct a phase II study evaluating the efficacy of rationally sequenced paclitaxel, gemcitabine, and carboplatin in patients with stage IV or select stage IIIB non-small cell lung cancer (NSCLC). METHODS: Patients with select stages IIIB (pleural effusion) and IV NSCLC with an ECOG performance status of 0-1 and no prior chemotherapy for their disease were eligible to participate. Treatment was delivered as follows: paclitaxel at 70 mg/m2 followed by gemcitabine at 300 mg/m2 on day 1, with carboplatin (AUC 5) on day 2 of a 28-day cycle. Response was assessed after every two cycles of therapy. The primary endpoint of this trial was response rate, with secondary endpoints of time to progression and 1 year overall survival. RESULTS: Twenty patients were enrolled on protocol, one of whom never received chemotherapy. The median number of cycles delivered was 3 (range 0-8). A partial response rate of 42% (8/19; 95% CI: 20-67%) and a stable disease rate of 11% (2/19; 95% CI: 1-33%) were observed. The median overall survival time was 9.6 months (95% CI: 4.6-16.6), with a 1 year overall survival rate of 42.1% (95% CI: 24.9-71.3%). Eight patients (42%) stopped treatment due to toxicity. CONCLUSION: Paclitaxel followed by gemcitabine and then carboplatin is an active, albeit complex, regimen in the treatment of patients with advanced NSCLC with insufficient advantage to justify continuation of this regimen.

In 1989, the University of Pennsylvania School of Dental Medicine (UPSDM) found there was an urgent need for new programs that would be effective in recruiting highly qualified and talented underrepresented minority students and ensuring their retention. Major efforts focused on leadership, financial support, institutional commitment, and the creation of an inclusive environment. UPSDM also offers an accelerated program leading to combined bachelor's and dental degrees and has agreements with several undergraduate institutions, including Xavier University in Louisiana and Hampton University, to enroll students in this program. UPSDM encourages minority retention through a Peer Mentorship Program, a Minority Mentorship Program, and course offerings that focus on diversity. Over the past thirteen years, these efforts have successfully garnered a fivefold increase in the number of underrepresented minority students at UPSDM.

Chemotherapy can cause a variety of physical and emotional changes that affect all aspects of patients' lives, including sexuality. Alterations in physical appearance can significantly influence people's perceptions of their sexual identities, attractiveness, and worthiness. Patients with lung cancer receiving chemotherapy may need sexual counseling. Therefore, patients and healthcare providers should create an environment that allows adequate time to discreetly discuss the impact that chemotherapy treatment may have on appearance, self-esteem, and sexuality. Nurses and physicians might hesitate to discuss sexuality with patients for various reasons, including time limitations, privacy considerations, readiness, and comfort level. Employers should provide support, educational programs, and professional resources so that healthcare providers can obtain the knowledge, develop the skills, and recognize that counseling patients about sexual issues is an important aspect in providing comprehensive holistic care to patients with lung cancer.

Free-ranging rhesus macaques (Macaca mulatta) were examined on the island of Cayo Santiago (CS), Puerto Rico, with intravenous glucose tolerance tests (IV-GTTs). Results were compared to monkeys derived from the island, but subsequently housed in corrals or gang cages at Sabana Seca (SS). Glucose clearance was similar among CS and SS macaques (range 2.99 to 4.50%min); both CS and SS males had significantly greater fasting serum glucose levels (63 ± 1 and 61 ± 2 mg/dl, respectively) than females; concentrations in CS and SS nonpregnant females (49 ± 3 vs. 52 ± 1 mg/dl) were significantly greater than CS and SS pregnant females (38 ± 3 vs. 41 ± 2 mg/dl) at each location. Fasting insulin was significantly less for male CS than for male SS macaques (means of 34 vs. 51 µU/ml); female averages were similar at both locations (range 48 to 61 µU/ml). The acute increment of insulin secretion for CS males was less than for SS males (85 vs. 136 µU/ml) but there were no differences between CS and SS females. The weight and body mass of CS macaques were generally greater than SS macaques. Hyperinsulinemic macaques > 10 years had greater weight and body mass compared to age-matched controls. Genetics may play a major role in expression of these impairments since the incidence on CS varied from 0 to 54% among the matrilines examined.
